This page is used to
validate your visa and
register your medical
insurance.
Validation
To keep
your visa from being
canceled by the SEVIS
Computer system, your
sponsor must validate your
visa within 2 weeks of the
start date on your DS-2019
form. At the time of
validation, sponsors are to
verify that the current U.S.
address of the exchange
visitor is correct.
SEVIS
will automatically terminate
the Form issued to an
exchange visitor if a
sponsor fails to validate
his/her participation in a
program prior to 30-days of
the program start date.
The
deadline for sending USHTA
information is 10 days from
your arrival date. Later
information could cause your
visa to become invalid.
Please
Complete the following
information then submit. Your
USHTA Responsible Officer will
officially register your
information on SEVIS with the
fee.
IF YOU MOVE DURING THE
PROGRAM:
You
are required to report to
USHTA if you move at any
time during your program.
You must update your contact
information within 10 days
of any changes. Please use
the form on this page to
report any changes that
should occur during your
program.
